The patient’s Dashboard is set up to show the most recent activity contributing to the patient’s individual care plan. In this case, daily measurements of blood glucose and blood pressure are taken. The patient also measures weight, and tracks feelings, steps, and medications as part of their prescribed treatment plan.
Information from the patient’s Diary is automatically available on CarePro, under the Owner profile. The patient receives ongoing support and reminders to help them stay on top of their care.

The patient receives a reminder to measure blood glucose. Notifications are visible on their device’s home and locked screens. Clicking on the notification automatically redirects to the New Entry screen, where measurements are logged using voice or manual entry. Glucose monitor readings can be synced from Apple Health directly into The Diary app.
The Diary’s Kindness Care® support team assists both patients and care team members in properly setting up their Diary to work seamlessly with Apple’s Health data. When this initial setup is complete, medical device data will automatically sync, eliminating the need for data entry.
Sometimes the simplest reminders can be the most important. It can be challenging to remember daily activities while managing chronic conditions. The Diary provides automatic notifications to pick up prescriptions, take medications, and attend appointments – which are all important for successful ongoing care.
Thresholds are set up in CarePro by the care team. Customizable Email, SMS, or Web notifications are sent when the patient’s measurements and data entries violate a threshold.
Assessments are set up in CarePro and sent to the patient’s Diary. The patient receives a notification and completes the Assessment, thereby eliminating the hassle of lost paperwork and onerous reporting.
